Appointment reminders are emailed one week before and the day before your scheduled appointment (via Patient Fusion online portal). At your initial scheduled visit, you will be directed to create an online login where you can input your health information as well as sign consent for treatment.
If you choose not to do the online intake, then please come into the clinic 15 minutes early to complete the forms for your initial visit. Expect your visit to last 60 minutes.
Please provide at least 24 hours notice if you are unable to keep your appointment, to avoid a fee of $80. There are many patients who may be able to take your spot if given enough notice! (Cancellations on a Monday should be made on the previous Friday.) When you miss an appointment with me, I not only lose your business, but also the potential business of other patients who could have scheduled an appointment for the same time.
