Appointment reminders are sent vis email and SMS text before your scheduled appointment (via Acubliss.com). At your initial scheduled visit, you will be directed to create an online patient account where you can input your HIPAA compliant personal and health information, sign consent for treatment and patient privacy policy, and set your messaging preferences.

If you choose not to do the online intake, then please come into the clinic 15 minutes early to complete the forms in-office for your initial visit. Expect your visit to last 60 minutes.

CANCELLATIONS
Please provide at least 48 hours business day notice if you are unable to keep your appointment, to avoid a fee of $110. There are patients who may be able to take your spot if given enough notice! As the clinic is closed on Friday, cancellations for a Monday appointment should be made on the previous Thursday.) When you fail to cancel an appointment, other patients who are on the wait list also lose out on the opportunity to take your spot.
